New Jersey Plastic Surgeon Philosophy

New Jersey plastic surgeon Dr. Friedlander believes that informed patients make the best surgical decisions. She listens to her patients carefully, then provides clear and comprehensive information so they approach surgery fully informed about their options. As a woman, she has firsthand knowledge of the issues and concerns facing her patients, and she brings to the table a level of comfort and ease that far exceeds that of her male colleagues. The decision to undergo plastic surgery is an important one and should never be rushed.

Expertise, Training, and Education

Dr. Friedlander is a Board Certified plastic surgeon with over 20 years of experience. Her practice focuses on cosmetic surgery of the breast and body contouring as well as procedures for facial rejuvenation. She is frequently called upon to perform “Mommy Makeovers”. These are procedures which are aimed at restoring a more youthful body, chest and face for women who have had significant changes in their appearance after childbirth and wish to feel better about themselves.

New Jersey plastic surgeon Dr. Friedlander grew up in Tennessee, but has resided in the Tri-State area for over three decades. She attended medical school at the State University of New York and completed a full general surgery training program in four years, rather than the usual, five. She continued with her plastic surgery training at the Albert Einstein College of Medicine, Montefiore Hospital in the Bronx before entering private practice in 1987. She is Board Certified in both general surgery and plastic surgery, although she limits her practice exclusively to plastic surgery. She has privileges at several local hospitals, including Overlook Hospital in Summit, NJ, Saint Barnabas Hospital in Livingston, NJ, and the Ambulatory Care Center in Livingston, NJ.

Professional Societies and Leadership

Dr. Friedlander is a member of the American Society of Plastic Surgery as well as the prestigious American Society for Aesthetic Plastic Surgery. She is a fellow of the American College of Surgeons and the International College of Surgeons and has remained active in these, and other, medical societies at the state and regional levels.

Dr. Friedlander has been on the staff of Overlook Hospital since entering private practice. She was Chief of the Plastic Surgery Section from 1996-1999 and currently serves as the Vice Chairperson of the Department of Surgery at Overlook Hospital.

As one of only a small percentage of Board Certified plastic surgeons who are women, she is firmly committed to ensuring that all women have access to breast health care. She was active in the American Cancer Society for many years working tirelessly in their Breast Outreach programs to ensure all underprivileged women and senior citizens have access to care and treatment. As a member of the Susan G. Komen Foundation speaker's bureau, she was a frequent speaker at both public and medical events and has been quoted in national and regional magazines as an expert in breast surgery.

New Jersey plastic surgeon, Dr. Friedlander has been honored by her peers as a “Top Doc” for many years. She has been listed in the Castle Conolly Guide to “Top Doctors in the New York Metropolitan Area” since 2000 and in New Jersey Monthly Magazine.

Cosmetic & Reconstructive Plastic Surgical Procedures

Cosmetic surgery gives you the ability to do more than simply change your appearance. It can transform the way you feel about yourself and the way you interact with the world around you. "Plastic" and "cosmetic" surgery covers a multitude of procedures, which alter and help to restore the body's outward appearance. Procedures are used to reshape areas, soften skin tone, and reduce or eliminate wrinkles.

Dr. Friedlander has a special interest in cosmetic breast surgery and body contouring. As a woman and mother of two grown children, she is able to understand the physical and emotional changes that occur naturally as a result of pregnancy or weight change. She can help to restore the breast to a more natural and/or youthful appearance using implants with or without a lift. She is uniquely qualified to address issues associated with post partum changes of the breast and body and is frequently called upon to perform “mommy makeovers”. These procedures involve combination surgery on the breast, abdomen and sometimes even face, which is aimed at restoring a more youthful figure. Liposuction of the thighs and hips may also be required for maximal improvement.

Dr. Friedlander also performs procedures directed at facial rejuvenation. These procedures such as eyelid lifts, neck lifts and forehead lifts are intended to restore a more youthful appearance. Her interest has recently focused on the “liquid facelift”. This procedure is ideal for the busy mom looking to refresh her appearance, and the professional women looking to maintain a competitive edge in this ever changing market. Botox will soften frown lines and eliminate a tired angry appearance and fillers will restore fullness to areas affected by gravity and time, such as the laugh lines and jowl regions.

She has performed all these procedures repeatedly, and is able to achieve a natural result that balances and enhances her patients existing features using state of the art techniques.

Descriptions about many of the procedures Dr. Friedlander performs have been provided so that you can educate yourself about the ones that interest you the most. After reviewing them, feel free to bring any remaining questions to your consultation.

Breast Augmentation/Enlargement/Enhancement

If you are like many women seeking breast augmentation, you have been considering the operation for many years. Some of you are looking to have natural appearing breasts that bring your body into balance while others are looking for a significant increase in size without that telltale bulge above. Breast augmentation is the 2nd most common plastic surgery procedure performed today. It is performed to enhance breasts which appear to be too small as a result of poor development, or post pregnancy shrinkage. It may also be used to correct breasts which are asymmetrical. The result can be not only enhanced physical appearance, but improved self esteem and inner confidence.

The procedures involve placement of an implant to enlarge the size of the breast. There are many different variables, such as location of the incision, the size, shape, and type of implant, as well as the location of the implant (under or over the muscle), which will be discussed with you. Dr. Friedlander prefers to place the implant behind the muscle, as this may decrease the potential for capsular contracture and will interfere less with future mammograms. She will discuss the implant alternatives of saline and silicone

After the surgery has been performed, it is normal to see swelling, specially high on the chest/breast. As the swelling disappears, the implants will look smaller. They are not. It is simply the swelling going away. The implants also settle into their position on the chest after several weeks. Most women are able to resume work 5-7 days after the procedure, but no strenuous activity is permitted for 4 weeks.

Breast Lift/Mastopexy

Pregnancy, weight change, and hormonal fluxes (menopause) can lead to breasts which are smaller in size and which sag or droop. A breast lift can correct this and restore the breast to a more youthful appearance by adjusting the nipple position and tightening the breast tissue. The areola (pigmented area around the nipple) is frequently reduced in size during a mastopexy. The cup size does not change with an uplift alone, but can be increased if the surgery is performed in conjunction with breast augmentation.

Dr. Friedlander will discuss the variables, including the size and shape of your breasts, as well as the condition of your skin. She will advise you of the best procedure. When only a small amount of sagging is present, the incisions can be made only around the areola. However, more frequently an incision will be required around the areola and vertically between the areola and the fold and finally in the fold itself. An implant can be inserted with any of these approaches. If the breast appears to sag, but the position of the breast and nipple are correct, then only an implant may be necessary. This distinction can be confusing and will be reviewed by Dr. Friedlander during your consultation.

Typically, there is mild discomfort after the surgery. This improves rapidly. However, you will be able to return to work after approximately one week. You should refrain from strenuous activity for four weeks.

Breast Reduction

Breast reduction is a wonderful procedure for eliminating symptoms of neck pain, back pain, shoulder pain, bra strap grooving, and recurrent irritation below the breasts caused by overdeveloped, disproportioned breasts. This procedure may be covered by your insurance carrier and a predetermination of benefits is obtained. The goal of the procedure will be to create smaller breasts, more in proportion with the remainder of the body, which will ultimately improve the symptoms listed above. It will also correct postural disturbances which have prevented you from comfortably working out or enjoying sports activities over the years.

This surgery is appropriate for teenagers with overly developed breasts that have stopped growing, as well as for mature women.

This procedure is done on a same day surgery basis. It involves elevating the nipple to a proper position and removing excess breast skin and tissue. Scars are placed around the areola, vertically extending to the fold, and within the fold itself. The scars will sometimes vary, depending upon the amount of tissue to be removed. In some instances, liposuction alone can be used to improve the breast size and eliminate symptoms.

Most women are sore for a few days after the procedure. They are able to return to work approximately 1 week after the surgery, and they must refrain from heavy or strenuous activities for at least 4 weeks after the surgery.

Breast Reconstruction

Breast reconstruction is one of the most rewarding surgical procedures performed by plastic surgeons. There are several different procedures which can be used to create the form and shape of a breast, which will closely approximate the opposite side. They will establish a more normal appearance and, more importantly, allow you to regain your self esteem. Today, breast reconstruction is usually initiated at the time of mastectomy. It may involve the creation of a breast mound entirely using your own tissue (TRAM flap) or it may involve using a tissue expander, which is a balloon placed temporarily under the chest muscle. This balloon is expanded or stretched as it is filled with saline. This stretches the skin slowly to create a pocket large enough to accommodate a permanent implant. There are also other procedures which use the skin and muscle of the upper back to protect an implant. All of these procedures are complex and will be reviewed with you in detail during your consultation with Dr. Friedlander. There will be plenty of time to ask questions and, as with all procedures discussed on this web site, she will show you photographs of each of the procedures.

Women of any age are entitled to look and feel whole after undergoing mastectomy. Breast reconstruction is covered by your insurance carrier. Breast reconstruction to improve asymmetry caused by a partial mastectomy (lumpectomy) may also be covered.

Other Breast Procedures

Some women are concerned about the appearance of their nipple and areola. If this is an area of concern, please discuss it with Dr. Friedlander. She can make the areola smaller, make the nipple protrude less or make it project more if desired.

Tummy Tuck/Abdominoplasty

Do you find that no matter how much you diet or work out you cannot eliminate your "pot belly" or "pooch"? A tummy tuck is a procedure that is aimed at correcting fullness of the lower abdomen that is caused by excess skin and fat resulting from pregnancy or weight gain and to correct areas of fat deposit that are resistant to dieting. The difference between this procedure and liposuction is that this procedure removes excess and/or stretched out skin in addition to tightening the underlying muscle (abs or rectus muscle) to give a flat, firm look. It may be performed with other contouring procedures or even GYN procedures, such as tubal ligation or hysterectomy.

Dr. Friedlander will review the benefits of a tummy tuck versus liposuction of the tummy. She will advise you which procedure will give you a better result. Sometimes it is beneficial to have liposuction of the hips and/or thighs performed simultaneously with a tummy tuck.

This surgery is done in the hospital and an overnight stay is required. You may wish to stay longer and this can be arranged. Dr. Friedlander will make an incision in your lower abdomen, which will permit tightening of the muscle and removal of excess skin. Two drains are used to prevent accumulation of fluid under the skin. They are removed after several days. Your bed is flexed and bent in the middle to take pressure off of your tummy. You will need to walk slightly stooped over and sleep with your upper body elevated on several pillows for about 1-2 weeks. You may return to routine activity after about 2 weeks and strenuous activity after about 4-5 weeks.

Contrary to popular belief, liposuction is frequently performed on men and women who are not overweight. These people are health conscious and diet and workout regularly, but have problem areas, such as their hips, thighs or love handles. Liposuction is the most common plastic surgery procedure performed today. It has different names and techniques. Suction assisted liposuction, tumescent liposuction, and power assisted liposuction all involve the same procedure. Ultrasonic liposuction uses sound waves, in addition, to remove fatty tissue. It removes fat in areas that are resistant to diet and exercise, even if you only weigh 100 pounds. The procedure also works well on patients who are over their ideal body weight and have localized fat deposits. It is not a substitute for weight reduction. It is ideally suited for the lower body but can also be used on the neck, arms, and even breasts in men (gynecomastia) and women (reduction mammoplasty).

The procedure is performed by placing small incisions (about ½ inch in size) in natural skin folds, where possible. A tumescent fluid is infused, which permits gentle removal of fat. Small cannulas (tubes) attached to a suction machine are passed repeatedly to remove the fat. The empty spaces once occupied by fat coalesce and flatten to recontour the region. Post operative swelling is normal and can last several weeks. A compression garment or girdle must be worn to help minimize swelling. Natural elasticity in the skin will permit tightening after the procedure. If this elasticity is lost, as can happen after pregnancy or with large shifts in weight, you may need a "tuck" in addition. No working out or strenuous activity is permitted for 4 weeks.

Thigh Lift

With aging, skin gets loose and flabby as natural elasticity disappears. In women, this tendency may result in large areas of saggy skin.

Thigh lifting is meant to stretch skin back into place, improve contours by making tighter, more attractive thigh and buttock skin. It will also reduce fat layers and decrease irregularities in the skin surface, as well as remove surplus skin.

Eyelid Surgery/Blepharoplasty

When we meet someone, we are often drawn to their eyes. This surgery treats the delicate areas around the eyes, which are the first areas to show signs of aging or fatigue. Eyelid surgery (blepharoplasty) removes excess skin and muscle, as well as protruding fat, from around the upper and lower eyelids. It can correct droopy upper eyelids and puffy bags below the lower eyelid. The exact procedure depends on your needs and the condition of the eyelids. Some people need only their upper or lower lids treated. Some may need both. Eyelid surgery can be done alone or in conjunction with other procedures used to restore a fresher, more youthful, appearance.

The surgery is performed in the hospital or ambulatory care center. When performed alone, it can be done under local anesthesia or general anesthesia, if desired. It is a same day surgery and you are followed closely in the office post-operatively. The incisions are placed within natural folds in the eyelid and usually heal exceptionally well and are not visible. The sutures are removed about 5 days after surgery. You are able to return to work in approximately one week.

Face Lift

A face lift is a procedure that can make your face look younger and fresher. It can turn back the hands of time by lifting your sagging cheeks, smoothing your jawline, tightening the loose skin of your neck, removing excess fat below your chin and improving a "turkey gobbler" appearance of your neck. It will improve the jaw line (jowls), changes in the neck that occur as a natural result of aging or heredity, and changes resulting from the skin's loss of tone and elasticity. Complimentary procedures involving skin care and ancillary services will help to further improve the negative effects of gravity, sun exposure and daily stress, as well as to help you maintain your result. Face lifts are chosen by men and women and can enhance self-confidence in those who decide to have them.

Dr. Friedlander will customize the procedure for you and address your areas of concern. You may choose to add eyelid surgery or other ancillary procedures to create a balanced look. The description which follows is the "classic" face lift. There are others, such as a neck lift and a mid face lift, which Dr. Friedlander also performs and will advise you if you are a good candidate for them. Dr. Friedlander will carefully review your medical history and the medications which you are taking. There are conditions which could conceivably cause problems during surgery such as uncontrolled high blood pressure, blood clotting problems, and others which will be discussed if they apply to you. If you smoke, she will counsel you on the increased risks associated with smoking. This is true for all surgeries, not only face lifts.

During the surgery, incisions are classically made in the hairline, as well as in front of and behind the ears. Newer techniques may not use an incision behind the ear. A small incision may also be made under the chin. These incisions result in minimal scarring which can be easily covered by hair or makeup. The skin and underlying fat are separated from the muscle. The fat may be thinned or suctioned to improve the overall contour of the face, neck and jaw line. The muscles of the face and neck will be tightened to provide longer lasting correction. The skin is lifted and redraped by pulling it upward and outward, and the excess skin is removed. Stitches around the ear are removed 5-7 days after the surgery, and stitches or staples hidden within the hair are removed 10-12 days after the surgery.

Face lift surgery has been revolutionized by "tissue glue". "Tissue glue" (fibrin sealant) is a relatively new and safe product in plastic surgery, although it has been used on over 6 million patients over the last 20 years for cardiac, neurosurgical, and general surgical procedures. Significantly less swelling and bruising occurs post-operatively. Messy drains and bulky dressings are rarely used today. Dr. Friedlander still advises you take 2 weeks off from work, but indicates that many of her patients are "lunching out" 2-3 days after the surgery. Some swelling and bruising are normal after surgery and begin to disappear after a few days. Strenuous activity must be limited for at least four weeks after surgery.

Forehead Lift/Brow Lift

A forehead lift addresses the area of the upper face. It restores a more youthful, refreshed look to the area, making you look less tired, angry, or sad, by correcting drooping brows and improving the horizontal lines on your forehead. It is frequently performed with an upper lid blepharoplasty and can be performed with a face lift to provide facial balance and harmony.

The goal of the procedure is to raise the brow to a more aesthetically appealing position. It will also improve fullness over the outside corner of the lid, a scowling appearance of the eyes and horizontal frown lines of the forhead. It is not intended to create a look of surprise or what is commonly called the "deer in the headlights" look. Botox is not a substitute for a forehead lift, it can give you an idea of what to expect. Dr. Friedlander will show you potential areas of benefit during your consultation.

Several different approaches may be used, such as an endoscopic approach, a lateral brow and transblepharoplasy approach and rarely a coronal approach. Most often, only small incisions are required. The procedure is often performed with face lift and/or blepharoplasty. There may be some postoperative swelling or bruising, which resolves relatively quickly. You are able to return to work in a week if no other surgery is being performed.

Other Facial Surgery

Cosmetic surgery of the face encompasses much more than has already been described. Many of the procedures discussed can be used as standalone procedures to enhance one's appearance and maintain facial balance. These include liposuction of the neck, chin augmentation (implants) and the use of facial fillers, such as fat, Juvederm, Radiesse, ArteFill to mention only a few. These products are FDA approved for use and recommendation will be individualized to fulfill your needs and goals.

Cosmetic surgery of the face frequently deals with problems associated with aging. One procedure that shouldn't be overlooked is surgery performed to correct protruding "Dumbo" ears. The problem develops in early childhood and can effect psychological development. Girls frequently wear their hair down to cover their large ears. A surgical procedure known as Otoplasty can improve this condition by reshaping the ears. An incision is placed behind the ear and the cartilage is folded and stitched, providing a more natural contour, closer to the head. A bandage is worn for several days after the surgery. The ears show immediate improvement and the swelling disappears after several weeks.
