BIOGRAPHY
 
Joshua A. Greenwald, MD grew up in Miami, Florida. While in high school in Miami, he was an all-American water polo player. In 1991, he earned his degree from Columbia University where he competed on the swimming and water polo teams. He was a member of the 1988-89 Eastern Intercollegiate Swimming League championship team. Dr. Greenwald graduated from the New York University Medical School in 1995. He remained at NYU for his general surgery training. During this time, he completed a two year fellowship at the NYU Institute of Reconstructive Plastic Surgery. His area of interest was gene
therapy of craniofacial abnormalities. Dr. Greenwald received numerous awards for his research contributions. He has published more than thirty peer-reviewed journal articles.
Dr. Greenwald was accepted into an extremely competitive fellowship in Plastic Surgery at Emory University in Atlanta, Georgia. The Division of Plastic Surgery at Emory is nationally recognized as one of the top training programs in the country.
With over 13 years of education and training since his undergraduate degree, Dr. Greenwald is highly skilled in cosmetic and reconstructive surgery. • Breast Augmentation (Augmentation Mammaplasty)
Breast augmentation with breast implants in Westchester County, New York by Dr. Greenwald involves placement of a saline or silicone implant behind the breast. Candidates for breast augmentation include women who seek to augment the size of their breast because they feel their breast size is too small, to correct a reduction in breast volume after pregnancy or weight loss, and to balance a difference in breast size. The implant is placed in a pocket created between the breast and the underlying pectoralis muscle (subglandular) or beneath the pectoralis muscle (submuscular). The implant is most commonly placed through minimal incisions located either in the fold beneath the breast (inframammary), at the junction of the areola with the skin of the breast (periareolar), or through the axilla (transaxillary). Most patients choose to have their breast implants placed through the inframammary or perioareolar approach as these incisions provide the surgeon with the most precise control over implant placement.

• Breast Reduction (Reduction Mammaplasty)
Patients with large, pendulous breasts may suffer from back, neck and shoulder pain related to the weight of their breast and the pull of their bra. Reduction mammoplasty, or breast reduction, is a surgical procedure to reduce the size and weight of the breasts in order to reduce these symptoms. It can also restore a more natural shape to the breast, and return the nipple to a more normal position on the breast mound. This procedure may be performed using a variety of incisions depending on the size and shape of the breasts. Dr. Greenwald routinely utilizes a wide variety of breast reduction techniques to help obtain optimal surgical results. Some of the newer techniques can give excellent results while limiting the length of the scar. This procedure is generally performed on an outpatient basis, and may be covered by your medical insurance plan.

• Breast Lift (Mastopexy)
Breast lift or mastopexy surgery in Westchester County, New York by Dr. Greenwald is a technique to restore a more youthful shape and position to a breast that has developed ptosis (drooping of the breast) related to aging, pregnancy, breast feeding, or weight loss. Over the years, these factors and the effects of gravity take their toll on a woman's breasts. As the skin loses its elasticity, the breasts often lose their shape and firmness and begin to sag. A breast lift is a surgical procedure to raise and reshape sagging breasts-- at least, for a time. (No surgery can permanently delay the effects of gravity.) Mastopexy can also reduce the size of the areola, the darker skin surrounding the nipple. Incisions required for breast lift are similar to those required for a breast reduction. The major difference between a mastopexy and a reduction are that in a reduction the excess breast tissue is removed while in a lift it is lifted and rearranged to a more youthful position. If your breasts are small or have lost volume, breast implants inserted in conjunction with a mastopexy can increase both their firmness and their size. Ideal candidates for breast lift or mastopexy surgery are those women who are pleased with the size of their breast when wearing a bra and those patients with good quality skin with few stretch marks.

• Breast Reconstruction
The goal of breast reconstruction surgery is to rebuild the contour of the breast so that it matches the normal cancer-free breast in symmetry and shape. Nipple areolar reconstruction and tattooing aims to restore symmetry between the nipples and areolas. Breast reconstruction has come a long way in the last twenty years. Most patients will get an excellent result with more than one technique.
Dr. Greenwald will help you choose the right technique for you. In addition to the technique chosen to perform your breast reconstruction, the timing of your breast reconstruction also merits some discussion. While most patients are candidates for immediate reconstruction (ie reconstruction at the same operation as your mastectomy), some patients will be better served by a delayed reconstruction. Though some women may not be interested in breast reconstruction, they should certainly be aware of their options. Finances should not be an issue when dealing with breast reconstruction. In 1998, the Women's Health and Cancer Rights Act was passed. This act mandates insurance companies to cover patients' procedure of choice in all cases of cancer patients who have had mastectomies, as well as surgery on the opposite breast to achieve symmetry.

• Gynecomastia Correction
Under certain circumstances, a man can develop fibro-fatty breast tissue that resembles a woman’s breast. This condition is referred to as gynecomastia. It is typically symmetrical enlargement of the breasts. The etiology of this process is not widely understood; however, it is known that certain medical conditions, medications, hormones, drugs, and other factors may play a role in its development. In the majority of cases, no etiology can be determined. Depending on the extent of the problem, gynecomastia may be treated with excision of the breast bud, liposuction of the fatty tissue, or a combination of both procedures. Occasionally, a secondary procedure to reduce excess skin or to return the nipple to a more normal position may be needed. Results can vary, but generally patients have a chest that is flatter, firmer, and better contoured, and they often feel more comfortable in form fitting clothing or going without a shirt.

• Liposuction (Suction Lipectomy)
Liposuction was the most popular cosmetic surgical procedure performed in the year 2005.  It is one of the more common procedures performed by Dr. Greenwald at his Westchester County, New York surgical facility.  Liposuction, also known as suction lipoectomy or lipoplasty, is a minimally invasive surgical technique used to eliminate localized fat deposits.  Traditionally, liposuction was used for recontouring the abdomen and hips; however, liposuction is now used in Dr. Greenwald’s practice to recontour and remove unwanted fat from just about anywhere on the body.  Refinements in liposuction techniques including the use of smaller cannulas allows
Dr. Greenwald to give you a precise result.   Liposuction can be dome as an ispolated procedure but may be combined with excisional surgery such as abdominopasty if indicated.

• Tummy Tucks (Abdominoplasty)
Abdominoplasty also known as tummy tuck was the fifth most common procedure performed by plastic surgeons in 2005. Dr. Greenwald routinely performs abdominoplasties and tummy tucks at his Westchester County, New York plastic surgery practice. Candidates for abdominoplasty are those patients with excess skin and fat of their abdomen. Basically there are three components to abdominoplasty: skin excision, liposuction, and muscle repair. The extent of each of these parts will be individualized for you. During abdominoplasty surgery, the excess skin and fat from the abdomen are removed, the underlying separation of the rectus muscles (“rectus diastasis” is repaired to tighten the waistline and excess fat is removed by a combination of direct excision and liposuction. Every effort is made to keep the resultant scar concealed in a patient’s
undergarment. • Mini-tummy Tucks (Mini-Abdominoplasty)
A mini-tummy tuck or mini-abdominoplasty is indicated in those patients who are not candidates for a full abdominoplasty and have a slight excess of skin which makes them poor candidates for a liposuction only procedure. A mini tummy tuck is a smaller procedure than a traditional abdominoplasty and is designed to primarily improve the contour of the abdomen below the belly button. A mini-abdominoplasty involves a shorter scar than a traditional abdominoplasty. It also involves more aggressive liposuction than may be safely permitted during a traditional abdominoplasty. Muscle repair is performed as indicated. A mini-tummy tuck is therefore the ideal treatment for a patient with minimal amount of excess skin usually overhanging a c-section scar with a moderate amount of extra fat deposits primarily in the lower abdomen. The belly button can be managed in one of two ways during a mini-tummy tuck. It can be left attached to the abdominal wall and will be pulled slightly downward as the excess skin and fat are removed form the lower abdomen. The belly button can also be “floated”. If a “float” is performed, the belly button is freed from the abdominal wall but left attached to the abdominal skin and relocated usually no more than 2 centimeters inferiorly. Compared with traditional abdominoplasty, there is minimal downtime with a mini-tummy tuck.

• Lower Body Lifts (Circumferential Abdominoplasty)
A lower body lift also known as circumferential abdominoplasty or belt lipectomy is indicated in those patients who may normally seek a traditional abdominoplasty but also have excess skin in their lateral thighs and buttocks. The lower body lift procedure improves the contours of the abdomen, lateral thighs and buttocks in a single operation. It is most commonly performed in patients that have had a significant weight loss from diet and exercise or from bariatric surgery. A lower body lift is an excisional procedure which removes excess skin and fat from the abdomen, thighs and buttocks. Results can be dramatic. The best candidates for circumferential body contouring procedures such as a lower body lift are those who have maintained a stable weight for at least six months.

• Thigh Lifts and Reductions
A lateral thigh and buttock lift is aimed at restoring a more youthful contour to these regions. Excess skin and fat are removed from lax areas and the skin and deeper supporting sturctures are tightened. It is often done in conjunction with a lower body lift to address soft tissue laxity of the abdomen, lateral thighs and buttock areas. Results can be dramatic. Fat harvested from another region of the body can be transplanted to the buttocks to create a fuller buttock if desired ("Brazilian Butt Lift").

• Arm Reduction (Brachioplasty)
A brachiplasty (arm lift) is  an operation aimed to improve contours of the upper arms through a combination of liposuction and direct excision.  Though some times it is done as an isolated procedure, it is typically performed in patients who have had significant weight loss.  In these patients, the arm lift or brachiaplasty is performed as part of a series of operations on the abdomen, thighs, buttocks, arms and breasts to recontour a patient’s entire body.  Redundant fat and skin removed from the upper arms is removed through an incision concealed in a natural crease where the arm meets the axilla (a short scar brachiaplasty). For patients with more severe laxity and tissue redundancy, an incision may need to be extended along the medial aspect of the arm.  All incisions are anchored to deeper structures of the thigh to prevent the final scar from spreading (ie a wide scar) and migrating.
